Creating and Editing a Form
Overview
It's easy to create and edit forms with different question types, branch logic, and styles, then preview what your form will look like for respondents! In addition, you can choose to make it a quiz, which comes with a few enhanced features. For more information, see the page on Quiz Options.
Note that MS Forms has two ownership options for forms and quizzes - personal and group.
- Personal – The form/quiz belongs to YOU and the data/files associated with it are stored in your personal OneDrive account.
- Group – The form/quiz belongs to one of your Microsoft 365 groups and the data/files associated with it are stored in the SharePoint site for that group.
- Visit the MS Forms website. If necessary, log in with your Tufts credentials.
- Pick an appropriate option – personal or group ownership:
- To create a PERSONAL form/quiz:
- At the top of the Forms homepage, click New Quiz or New Form depending on what you want.
- A new form/quiz will load.
- To create a GROUP form/quiz:
- Scroll to the bottom of the Forms homepage.
- Find the group that you want the form/quiz to belong to and click on it.
- Click New Group Quiz or New Group Form.
- A new form/quiz will load.
- To create a PERSONAL form/quiz:

- Visit the MS Forms website. If necessary, log in with your Tufts credentials.
- Depending on who owns the form, you can access it in different ways.
- A personal form that belongs to you –
- Select My forms.
- Find the form and click on it. The form/quiz will load.
- A group form that belongs to a group that you are a part of –
- Scroll to the bottom of the Forms homepage.
- Find the group that you want the form/quiz to belong to and click on it.
- Find the form and click on it. The form/quiz will load.
- A form that belongs to someone else and you were invited to as a collaborator –
- Select Shared with me.
- Find the form and click on it.The form/quiz will load.
- A personal form that belongs to you –

- Visit the MS Forms website. If necessary, log in with your Tufts credentials.
- Create a new form or access an existing one using the directions in the previous sections.
- The form will open in edit mode. Some of the things you can do include:
- Edit title and subtitle
- Create new questions
- Edit existing questions
- Edit question options
- Copy, delete, or move questions

Branching allows you to determine how respondents flow through your form/quiz, depending on how they answer particular questions.
- Click on the Options button for a question and select Add branching. A “Branching options” page will load.
- Click into a question/section to manage branching. Depending on the section and the question(s) it contains, you may be able to pick where respondents are directed to after selecting certain responses or completing the entire section.
- Click Style in the top toolbar.
- Options will be presented for form design, colors, animations, and even background music.

NOTE: While in preview mode, you can click through your form and fill it out! Be aware that if you do this, the data that you generate will be saved to the form’s responses!
While editing a form, click the Preview button in the top toolbar.

A preview window will open with options for both Computer and Mobile layouts.
