Microsoft Forms: Online Surveys

Microsoft Forms is just one of several tools available at Tufts for collecting data and digital signatures through electronic forms! For more information on these tools and how they compare, visit the Form Building FAQs page on AccessTufts.

Microsoft Forms is an electronic form tool that allows users to:

  • Create basic forms and quizzes
  • Distribute to your desired respondents
  • View form data, including pre-made visualizations
  • Collaborate with other people on form management

Forms/quizzes can be owned by specific individuals, or by groups for easier collaboration.

When you visit the Microsoft Forms website, the homepage will look similar to what you see below.

(A)Create a new personal quiz or form from scratch or import from Word/PDF

(B)Start from a template

(C)Filter/Access personal forms and quizzes, including ones that you own, ones that you previously filled out and saved, and ones that you were invited to collaborate on

(D)Create and access group forms

Although this guide focuses on the use of Microsoft Forms through the forms website, MS Form functionality is also integrated into other tools as well.
